Our facility is committed to providing quality and compassionate nursing and rehabilitation services in a safe, comfortable environment. We strive to make your “home away from home”, whether a short or long-term need, a satisfying healing experience. Our nursing, therapy and interdisciplinary care team members have extensive and diverse clinical expertise that meet the wide range of chronic and acute conditions of individuals who are not quite ready to return safely home following a planned medical intervention or unexpected medical complication. Our team is equally focused on meeting the long-term holistic needs of those who choose to make our facility their permanent residence. We accept admissions 24 hours a day, 7 days a week, complemented by around the clock nursing care and daily specialized rehabilitation services. We are excited to be your local leader in nursing & rehabilitation services.

Our Therapy Department at The Woodlands recently teamed up with our Nursing Team for a hands-on Skills Fair! 👏 Together, staff practiced safe transfers, proper body mechanics, correct gait belt placement and safe patient mobility.

Our PT and OT teams are working together to help residents build strength, improve mobility, and gain greater independence! 🤝

Through co-treatment sessions, residents can practice real-life skills like walking, reaching, balance, and everyday activities, all while improving coordination, confidence, and safety. 🚶‍♀️❤️

Every step forward brings our residents closer to their goals and a safer, more independent return home! 🏡✨

When Mr. Butler arrived at The Woodlands following a hospitalization, he was unable to walk or safely care for himself at home. Through determination, hard work, and the support of his therapy team, he made incredible progress! 💪🏼❤️

Today, we’re proud to celebrate Mr. Butler as he returns home to continue his rehabilitation journey with Home Health! 🏡🎉
