Our facility is committed to providing quality and compassionate nursing and rehabilitation services in a safe, comfortable environment. We strive to make your “home away from home”, whether a short or long-term need, a satisfying healing experience. Our nursing, therapy and interdisciplinary care team members have extensive and diverse clinical expertise that meet the wide range of chronic and acute conditions of individuals who are not quite ready to return safely home following a planned medical intervention or unexpected medical complication. Our team is equally focused on meeting the long-term holistic needs of those who choose to make our facility their permanent residence. We accept admissions 24 hours a day, 7 days a week, complemented by around the clock nursing care and daily specialized rehabilitation services. We are excited to be your local leader in nursing & rehabilitation services.

When Mr. Fleming arrived at The Woodlands following a hospitalization, he was weak and unable to care for himself independently. Through determination, hard work, and the support of our dedicated therapy team, he regained the strength and confidence needed to return home.
While his rehabilitation journey continues with home health therapy, reaching the milestone of going home is a tremendous achievement. We are so proud of the progress he has made and are honored to have been part of his recovery.

When Mr. Stacey first arrived at The Woodlands following a devastating car accident, he required maximum assistance with nearly all activities of daily living and was unable to walk.
Through determination, perseverance, and countless hours of hard work alongside our therapy team, he continued to make remarkable progress every step of the way.
Today, we’re celebrating an incredible milestone, Mr. Stacey walked out of The Woodlands and returned home to his family! ❤️ He will continue building on his success through outpatient therapy, and we know his journey is only getting stronger.

🎉 Congratulations to Mr. Small! 🎉
When Mr. Small arrived at The Woodlands, he required maximum assistance and was barely able to stand. Through hard work, determination, and dedication to his therapy program, he made incredible progress during his stay.
Recently, Mr. Small was able to take several steps using his new prosthetic, a huge milestone on his rehabilitation journey! 👏
