{"id":86,"date":"2020-10-12T13:32:25","date_gmt":"2020-10-12T13:32:25","guid":{"rendered":"http:\/\/www.nexion-health.com\/thibodaux-2020\/?page_id=86"},"modified":"2021-01-13T23:41:54","modified_gmt":"2021-01-13T23:41:54","slug":"moving-in","status":"publish","type":"page","link":"https:\/\/www.nexion-health.com\/thibodaux-healthcare\/maison-bienvenue\/assisted-living\/moving-in\/","title":{"rendered":"Moving In"},"content":{"rendered":"<p>You may find this information helpful in making the transition to your new home.<\/p>\n<ol>\n<li>Forms you will need to complete and bring:\n<ul>\n<li>Physician&#8217;s orders form<\/li>\n<li>Medications currently prescribed<\/li>\n<li>CPR\/advanced directive forms<\/li>\n<li>Copy of power of attorney or guardianship, if applicable<\/li>\n<li>General information sheet<\/li>\n<li>Resident assessment (to be completed with our nurse)<\/li>\n<li>Move-in summary fee sheet<\/li>\n<li>Deposit check<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"2\">\n<li>Identification cards (we will copy and return the originals to you)\n<ul>\n<li>Medicare card<\/li>\n<li>Social Security card<\/li>\n<li>Medicaid card<\/li>\n<li>Any other medical insurance cards<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"3\">\n<li>Arrange 10 days before your move-in date, if possible:\n<ul>\n<li>To order newspaper contact The Daily Comet at 985-448-7600<\/li>\n<li>Change of address with the post office<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"4\">\n<li>Bring with you:\n<ul>\n<li>Furniture and mementos<\/li>\n<li>Clothing and clothes hangers<\/li>\n<li>Personal items including grooming supplies (all bathroom personal items)<\/li>\n<li>Kitchen items you may need<\/li>\n<li>Telephones and cords (if using a land line)<\/li>\n<li>TV and cable cord<\/li>\n<li>Pictures and picture hangers<\/li>\n<li>Wheelchair or walker, if necessary<\/li>\n<li>Shower curtain<\/li>\n<li>Bath rugs (be wary of them due to trip hazard)<\/li>\n<li>Plunger and toilet brush<\/li>\n<li>Laundry supplies<\/li>\n<li>Curtains or toppers and rods, and hardware to hang them, if desired (mini-blinds are supplied)<\/li>\n<li>Personal linens, blankets, bedspreads, and pillows, if desired<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<a href=\"https:\/\/www.nexion-health.com\/thibodaux-healthcare\/wp-content\/uploads\/sites\/63\/2021\/01\/Maison_Bienvenue_Move_In_Checklist.pdf\" class=\"button \" aria-label=\"link leading to https:\/\/www.nexion-health.com\/thibodaux-healthcare\/wp-content\/uploads\/sites\/63\/2021\/01\/Maison_Bienvenue_Move_In_Checklist.pdf\" role=\"button\">Download Our Move-In Checklist<\/a>\n","protected":false},"excerpt":{"rendered":"<p>You may find this information helpful in making the transition to your new home.
