Move-In Checklist
You may find this information helpful in making the transition to your new home.
- Forms you will need to complete and bring:
- Physicians Orders Form
- Medications currently prescribed
- CPR/Advanced Directive Forms
- Copy of Power of Attorney or Guardianship, if applicable
- General Information Sheet
- Resident assessment to be completed with our nurse
- Move-in Summary Fee Sheet
- Deposit Check
- Identification Cards (we will copy and return the originals to you)
- Medicare Card
- Social Security Card
- Medicaid Card
- Any other medical insurance cards
- Arrange 10 days before your move-in date, if possible:
- Phone transfer with telephone company

- AT&T Telephone (for land line hook up) 885-951-0534
- To order newspaper contact 985-384-8370
- Change of Address with the post office
- Bring with you:
- Furniture & Mementos
- Clothing & clothes hangers
- Personal items including grooming supplies (All bathroom personal items)
- Kitchen items you may need
- Telephones and Cords (if using a land line)
- Cable cord for TV (if cable connected)
- Pictures and picture hangers
- Wheelchair or walker, if necessary
- Shower curtains, liners, and rings
- Bath Rugs (be wary of them due to trip hazard)
- Plunger and toilet brush
- Laundry Supplies
- Curtains or toppers & rods, and hardware to hang them, if desired (Mini-blinds are supplied)
- Personal linens, blankets, bedspreads, and pillows, if desired
