Rehabilitation
Our therapy professionals are skilled in designing individualized rehabilitation recovery plans to ensure you or your loved one’s home health experience is effective, safe, and satisfactory. We offer Physical Therapy, Occupational Therapy, and Speech Therapy services.
Home Health Aides
Nexion at Home personal home aides can offer assistance for your loved one with activities of daily living including personal care, bathing, grooming, dressing, ambulation, and light housekeeping while in the comfort of your own home.
Virtual Care
Nexion at Home proudly offers virtual visits and telemedicine technology in addition to in-person visits from clinicians. A team member will facilitate the video meeting, allowing for real-time ability to addresses changes in condition with immediate care.
Social Services
A Social Services team member assists with coordination of care, streamlining care and assisting patients and caregivers in the management of their health.
