To better serve our residents, we offer a progressive approach to treatment that combines collaboration with primary physicians and individualized rehabilitation services with resident and family education.

Residents who are part of our wound management program will have access to a team of professionals with specialized training and experience. Our Director of Nursing and physical therapy teams work with the resident’s primary care physician to put together an individualized care plan that best meets their needs.
Our affiliate facilities provide residents and their caregivers with education on their disease process and provide them with a comprehensive plan of care for wound healing. Through an interdisciplinary approach supported by certified wound care specialists, our residents can achieve wound healing and the ability to sustain a healthy lifestyle.
Our affiliated administrative nurses and physical therapists go through rigorous education and training on the current best practices for prevention and the healing of skin conditions.
Our affiliates take pride in developing personalized plans of care with the goal of preventing the development of pressure ulcers and other skin conditions, as well as expediting the healing of existing wounds.

Our affiliates aggressively assess a resident’s skin and respond to changes in condition timely to ensure the plan of care is adequate for prevention and optimal wound healing.
An individualized plan of care is developed for each patient that will include:

Our affiliated facilities are supported by experienced and certified staff in the areas of skin and wound care, utilizing technology like Facetime and other conventional tools to expand access for real-time bedside consulting.
We utilize debridement and Doppler competent physical therapists for in-house advanced wound care services who are supported by a certified wound care specialist PT for consultation and competency validation.

Nursing teams can manage both PICC and central lines.
Total Parenteral Nutrition (TPN) is used when individuals cannot or should not get their nutrition through eating. TPN provides nutrition to the body by bypassing the digestive system and dripping a nutrient solution directly into a vein.
TPN is a mixture of nutrients, such as carbohydrates, proteins, lipids (fat), electrolytes, vitamins, and minerals. The solution may have all or some of these nutrients, based on what the resident’s body needs. The solution may also include certain medications.
The administration of TPN is complex, and our affiliates’ nurses are trained and in-serviced by our experienced clinical team with the knowledge of parenteral nutrition, as well as the central venous access devices that are used to administer them. Our affiliates use an interdisciplinary approach to providing the resident with proper nutrition through TPN.
Dialysis is a treatment that removes harmful substances from the blood when the kidneys cannot. There are two main types of dialysis: hemodialysis and peritoneal dialysis. Hemodialysis uses a special type of filter to remove excess waste products and water from the body. Peritoneal dialysis uses a fluid that is placed into the patient’s stomach cavity through a special plastic tube to remove excess waste products and fluid from the body. Both types of dialysis are provided to our patients.
Dialysis can be a tiring and stressful process. By offering this service at select Nexion affiliated facilities, we eliminate the need for transportation to a clinic, thereby enhancing comfort and convenience for our residents. Our continuity of care helps ensure the dialysis and nursing staff will be consistent during the resident’s stay.
Our care team has advanced knowledge and training in a variety of pain management techniques, so they can provide quality care to our residents.
After comprehensive evaluation and review of medical history, our staff will work with the patients and their primary care physician to develop a personalized treatment plan that best fits their needs. Our goal is to reduce pain and suffering so our patients can move towards a more productive and comfortable life.

Nexion Health and its affiliates are taking steps to bring medicine and experts to our residents, which supports both a culture of “care in place” and our efforts to provide responsible and reasonable healthcare services. We do this through the utilization of telemedicine to: 1) enhance the satisfaction of our stakeholders (our residents, families, staff, physicians and referral sources); 2) advance integration of information technology; 3) improve resident quality of life; 4) effectively manage return to acute (RTA) or re-hospitalization while promoting a culture of “care in place”; and finally 5) reduce overall healthcare spend by reduction in incidence of and severity of common hospitalization diagnoses and associated conditions/co-morbidities.
Telemedicine provides real-time physician/extender access to the resident, reflecting the culture of “care in place”- which includes enhanced physician support to the direct care staff and residents through routine daily access and enhanced nocturnal and weekend coverage. Access to specialists/consultants is improved through telemedicine reducing the time between clinical concern and physician consultation. This reduction in time results in efficient, enhanced care delivery, increased stakeholder satisfaction and reduced per beneficiary cost by preventing unnecessary hospitalizations as well as reducing severity of the identified clinical condition and/or complexity of clinical change in condition.
A telemedicine event is a HIPAA-compliant consultation coordinated with each resident’s primary physician, direct care staff and consulting specialists.
